One of the problems is that today’s modern construction methods have replaced lightweight PVC in lieu of cast iron drains. Although it is cheaper and very durable, PVC is a very poor sound blocker.
The solution is to wrap the outside of the offending pipes or ductwork with a foil backed rolled composite product that combines a fiberglass absorber with a mass loaded vinyl barrier to effectively decouple and block noise.
Simply cut to size, wrap pipe or duct overlapping a few inches until snug without compressing the fiberglass, then tape in place with matching foil tape. Problem solved!
